Charges are based on the type and cost of the services required, the time of the professionals who treat you, and the complexity of your medical problem. Your total charge will reflect this work and time, and may combine charges for both the physician and hospital services, such as laboratory work. You will be asked to pay your deductible and co-insurance at the time of your visit. Claims will be processed with your insurance carriers for services rendered, but you are responsible for payment regardless of your insurance coverage or claims handling. If you are covered by a managed care plan, you will need to bring a referral form, pre-certification authorization, and/or authorization number. |
