For new patient appointments, please bring the following information:

• Insurance information;
• Prescription medications you are currently taking;
• List of any know food or drug allergies;
• Names and addresses of any physicians you would like to receive reports from your visit.

To provide sufficient time for registration, plan to arrive 30 minutes before you scheduled appointment. When you arrive for your appointment you will be asked to fill out a questionnaire. Parking is available in the Duke Clinic Parking Garage on Trent Drive. Wheelchair assistance is available upon request at the Duke Clinic drive-up entrance. Shuttle service between the Duke Clinic and the parking garage is available Monday through Friday from 7:30 am until 5:00 pm.
